BACKGROUND: Early childhood caries is a significant public health concern affecting about 600 million children globally. The etiology of early childhood caries can be explained as an interplay between genetic and environmental factors. Single nucleotide polymorphisms are the most common variations in the human genome. Genetic variations of immune response genes can modify the defense response of the host, and alter the susceptibility to bacterial colonization of the oral cavity and early childhood caries. The aim of this systematic review is to identify genetic variants of immune response genes associated with early childhood caries. RESULTS: A total of 7124 articles were identified by conducting an elaborate search across various electronic databases and genome-wide association studies databases. Subsequent to exclusion at various stages, fifteen articles qualified to be included into the present review. Risk of bias assessment was done with the Q-genie tool. Quantitative synthesis revealed that the odds ratio for TT and CC genotypes of rs11362 was 1.07 (0.67-1.71) and 1.16 (0.84-1.60), respectively. Gene-based analysis revealed a statistically significant association between variants of tumor necrosis factor-alpha gene and T-cell receptor alpha variable 4 locus with early childhood caries. Gene clustering showed the presence of three functional clusters. To comprehend the protein-protein interaction, the bioinformatic tool of "Search Tools for the Retrieval of Interacting Genes and Proteins" was used. Among the biological processes and the reactome pathways, complement activation through the lectin pathway showed the highest strength of association with early childhood caries. To understand the interaction and functionality of the genes, "gene function prediction using Multiple Association Network Integration Algorithm" was used, which revealed that the genes were linked by physical interaction (39.34%) and through co-expression (34.88%). CONCLUSIONS: Genotype TT of rs7217186 of arachidonate 15-lipoxygenase gene was a risk factor for early childhood caries. Multiple genetic variants of T-cell receptor alpha variable 4 locus and tumor necrosis factor-alpha gene were associated with increased susceptibility to early childhood caries. Polymorphisms of genes regulating the lectin pathway of complement activation can modify the susceptibility to early childhood caries.

Introduction: Genetic polymorphisms of genes regulating amelogenesis can alter susceptibility to Early Childhood Caries (ECC). This systematic review aims to analyze associations between single-nucleotide polymorphisms of enamel formation genes and ECC. Methods: Search was conducted across PUBMED, CINAHL, LILACS, SCOPUS, EMBASE, Web of Science, Genome-Wide Association Studies databases from January 2003 to September 2022. This was supplemented by hand search. Totally 7124 articles were identified and 21 articles that satisfied the inclusion criteria proceeded to data extraction. Quality assessment was done using the Q-Genie tool. Results: Quantitative synthesis revealed that homozygous genotype AA of rs12640848 was significantly higher in children with ECC with an odds ratio of 2.36. Gene-based analysis revealed significant association between six variants of AMBN, four variants of KLK4, two variants of MMP20, and a single variant of each of MMP9 and MMP13 genes and ECC. The Bonferroni corrected-log10 P value of amelogenesis gene Cluster was 2.25 (0.05/88 = 5.6 × 10-4). Search Tool for Retrieval of Interacting Genes and Proteins plot constructed to comprehend the protein-protein interaction revealed the presence of four functional clusters. Gene function prediction using Multiple Association Network Integration Algorithm revealed that physical interaction between these genes was 69.3%. Conclusion: Polymorphisms of genes regulating amelogenesis can influence the susceptibility to ECC. AA genotype of rs12640848 may increase the susceptibility to ECC. Gene-based analysis revealed a significant association between multiple polymorphisms of genes regulating amelogenesis and ECC.

Introduction: The aim of this systematic review was to assess the antimicrobial activity of nanoparticle-mediated photodynamic therapy (N-PDT) on Enterococcus faecalis biofilms in the presence of dentin substrate when compared to photodynamic therapy (PDT) and conventional disinfection protocols. Materials and Methods: This systematic review was registered in Open Science Framework (10.17605/OSF.IO/GBR3F). Six databases, namely PubMed, Embase, Web of Science, Scopus, Medline, and Google Scholar, were searched for English language articles until June 2022. Laboratory studies assessing the antimicrobial activity of N-PDT against E. faecalis biofilm in human or bovine teeth were included. The risk of bias (RoB) was evaluated using the Joanna Briggs Institute tool for quasi-experimental studies. Meta-analysis was performed using the random-effects maximum likelihood model. Results: The search revealed 2804 articles, out of which 9 studies were included in the final review. Seven articles had low RoB and two had moderate RoB. Chitosan and diode laser at 810 nm were the most commonly used nanoparticle and light source, respectively. The meta-analysis of bacterial reduction log and percentage reduction revealed that N-PDT had better antimicrobial efficacy than the control group. When the bacterial reduction log of N-PDT was compared with PDT, PDT performed better N-PDT, and for percentage reduction, there was no difference. Conclusion: The currently available evidence is low and inconclusive with regard to the superior efficacy of N-PDT. The type of nanoparticle, incubation time, light source, and exposure time were found to be covariates that influence the antimicrobial efficacy of N-PDT.

INTRODUCTION: This systematic review and meta-analysis was performed to evaluate the association between pulp calcifications (PC) and cardiovascular disease (CVD). METHOD: Five databases (PubMed, Scopus, Embase, Web of Science and ProQuest) were searched to identify articles. Quality assessment of the selected articles was done using Joanna Briggs Institute critical appraisal tool and the inter-examiner agreement was calculated using the Cohen-Kappa test. The random-effects maximum likelihood model was used to evaluate the association between the conditions. Trim-and-fill funnel plot was used to evaluate the presence of publication bias. The level of evidence was evaluated using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) tool. RESULTS: A total of 2487 articles were identified from the initial search. Ten articles were included in this review of which 7 were cross-sectional and 3 were case-control studies. Eight articles had a low risk of bias (RoB), one article each had a moderate and high RoB. Inter-examiner reliability score was 0.8592. Seven articles reported a statistically significant positive association between CVD patients and PC. Two articles reported a non-significant positive association and one article reported no association between the same. A significant difference was observed in favor of CVD patients to possess more PC in comparison to non-CVD patients with an OR of 4.30 (95% confidence interval (CI): 2.19, 8.46; p = .000). Asymmetry in the Trim-and-fill funnel Plot suggested the presence of publication bias. GRADE analysis demonstrated low certainty of the evidence for the overall sample. CONCLUSION: There exists a low level of evidence to show that CVD patients are more prone to have PC in comparison to non-CVD patients.

Aim The aim of this systematic review and network meta-analysis was to identify the best irrigant activation technique (IAT) for the removal of accumulated hard tissue debris (AHTD) from the mesial root of mandibular molars evaluated using micro-CT studies.Methods The research question was based on the PICO format. Four electronic databases - PubMed, Scopus, Embase and Web of Science - were searched for articles up to June 2020. Selected articles were assessed for bias using the Joanna Briggs Institute Critical appraisal tool. The network meta-analysis using a fixed-effects model and SUCRA ranking were performed. The quality of the evidence was assessed using the CINeMA framework.Results Eleven studies were included for qualitative synthesis while seven were included for quantitative synthesis. The risk of bias of all included articles was low. The results based on SUCRA values revealed the IAT shock wave-enhanced emission photoacoustic streaming (100%) resulted in the greatest reduction of the volume of AHTD from mesial roots of mandibular molars. Ranking of the other IATs was as follows: photon-induced photoacoustic streaming (87%), laser-activated irrigation (79.3%), XP EndoFinisher (71.2%), ultrasonically activated irrigation (59.6%), apical negative pressure (42.3%), EasyClean (37.4%), EDDY (26.2%), EndoActivator (24.2%), self-adjusting file (11.6%) and needle irrigation (11.3%).Conclusion None of the IATs rendered the root canals completely free of AHTD. The laser-activated irrigation groups fared better than all other interventions in reducing AHTD from the mesial roots of mandibular molars. The confidence rating ranged from low to high for indirect evidence and moderate to high for mixed evidence. Results must be interpreted with caution due to the laboratory nature of the included studies.

INTRODUCTION: The purpose of this review was to determine the diagnostic accuracy of cone-beam computed tomographic (CBCT) imaging in detecting vertical root fractures (VRFs) in root-filled teeth compared with a reference standard (direct visualization). METHODS: Electronic searches were performed in Medline, Scopus, Cochrane, and gray literature for English language articles until June 2020. Prospective and retrospective clinical studies using CBCT imaging to diagnose VRFs in root-filled teeth were included. Case reports and in vitro studies were excluded. The Quality Assessment of Diagnostic Accuracy Studies 2 tool was used to assess the risk of bias and applicability concerns. Meta-analysis was performed using Stata 16.1 software (StataCorp, College Station, TX) via the MIDAS v.3.0 package and METANDI module. Publication bias was evaluated using Deeks' funnel plot analysis. Grading of Recommendations, Assessment, Development, and Evaluations (GRADE) was performed to evaluate the certainty of evidence. This systematic review was registered in the Open Science Framework (10.17605/OSF.IO/7JKE2). RESULTS: Eight articles were included in this systematic review and meta-analysis. Risk of bias assessment showed that 5 articles in the patient selection domain had low risk of bias with low applicability concern. In the index test and reference standard domains, 7 articles had moderate risk of bias with moderate applicability concern. Three articles had high RB in the flow and timing domain. There was no publication bias. CBCT imaging had a pooled sensitivity and specificity of 0.78 (95% confidence interval [CI], 0.64-0.88) and 0.80 (95% CI, 0.63-0.91), respectively, and an accuracy of 0.86 (95% CI, 0.83-0.89). CBCT imaging also had pooled positive and negative likelihood ratios of 4 and 0.2, respectively. In GRADE analysis, the quality of evidence was low for sensitivity and moderate for specificity when CBCT imaging was used for the diagnosis of VRF. CONCLUSIONS: The overall quality assessment of the included articles showed that in the patient selection domain, the risk of bias was low, and it was moderate in the index test and reference standard domains. Evidence from this systematic review and meta-analysis indicates that CBCT imaging is still not a good tool for diagnosing VRFs in root-filled teeth compared with direct visualization.
